Application forms PSC Form 3 (Revised 2008) are obtained from Secretaries of District Service Commission country wide as well as Public Service Commission. Applicants must attach photocopies of their Certificates, Transcripts together with (3) three recent passport photographs and a photocopy of National Identity Card.
